Waiver & Code of Conduct Template

As a youth leader, it is important to have the families at your parish sign a waiver and code of conduct before the ministry year. This way, you know you have the proper permission to care for their children, as well as are aware of any medical conditions any of the youth have, and have a standard for behavior at a youth ministry event. See below waiver and customize it to your parish and needs.

Creating a Youth Ministry Calendar
